Thez's amulet is enchanted with a unique form of the polymorph spell. While wandering the wilds of the surface, he accidentally happened upon it, he thinks it's a simple disguise spell that is preset to a specific sun elf appearance.

In truth, it can not transform the wearer into any creature, but was made to be a type of magic prosthesis. The amulet attempts to aesthetically revert external damage to mirror what was lost, and once the amulet is removed, the prosthesis dismisses and the damage returns.

Due to ceremorphosis changing the property of human flesh to Illithid, the amulet sees Thez's body as one large scar and attempts to change it as best as it can. Because of this, Thez takes on the visage of his host, even though they died long ago so that he could be born.
